You have the right to make decisions about the health care you get now and in the future.
Federal law requires that you be told of your right to make an advance directive when you are admitted to a healthcare facility. Illinois law allows for the following three types of advance directives: healthcare power of attorney; living will; and mental health treatment preference declaration.
Advance directive packets are available as a printed packet at all Carle locations. The information in this packet is also available as a series of fillable pdf documents. You will need to sign and obtain any required witness signatures before giving a copy to Carle. The documents provide clear guidelines above the witness signature line(s) regarding who is eligible to witness.
Your Advance Directives can be attached to your Carle medical record. You can give a copy of your Advance Directives to your nurse during your next appointment. You can also mail completed documents to:
