JOB SUMMARY:
Provides supportive services, case management, education, counseling, review and reporting services to residents of this Federal Bureau of Prisons Residential Reentry program

CERTIFICATION & LICENSURE REQUIREMENTS

ESSENTIAL FUNCTIONS:
  • Key Accountability - Provision of Supportive and Case Management Services* Provides residential reentry services for individuals placed by the FBOP in the residential reentry program to overcome barriers to successful community reintegration.* Completes comprehensive intake services at the time of placement at RRC* Conducts orientation of the residents to the RRC environment* Provides a comprehensive assessment of risks and barriers to successful reintegration* Develop, with input from residents, comprehensive individualized service plan which identifies areas of risk and goals, methodologies and interventions to intervene in risk areas* Requests medical, psychiatric and substance abuse assessment/consultation, where indicated, in a diligent and timely manner in coordination with the FBOP.* Provides individual and group services to include, but not limited to substance abuse aftercare, monitoring of daily living activities, assistance procuring benefits and entitlements, assistance with employment and education, support in goal attainment, skills training, assistance accessing resources, family support, coordination of services and care.* Provide weekly group interventions in the areas of drug aftercare, general house meetings, employment seeking/keeping/coping, family reintegration, and transition services* Maintains records in accordance with TrilliumPlace and FBOP Statement of Work and policies and procedures. Completes bi-weekly progress reports.* Acts as a liaison and resource between the community and residents' needs; coordinates individualized program goals with community agencies, primary care physicians, United States Probation office
  • Key Accountability - Provision of Review and Reporting Services * Update all status changes for all residents into the FBOP R3M system and submit documentation to the FBOP on progress towards goal attainment and Home Confinement completion* Complete approval process of pass sites and discharge sites by conducting a site visit to the pass or discharge residence and interviewing the site residents and complete employment site initial approvals* Complete monthly pass and employment site unannounced checks for home confinement residents, monitor GPS tracking of all home confinement residents and approve weekly itineraries for all home confinement residents to be in compliance with their itinerary* Complete incident reports for residents violating FBOP guidelines while at the RRC and facilitate formal investigations and hearings and submit all findings to the FBOP-DHO on all incident reports* Monitor and evaluate all residents based on the directives of the Prison Rape Elimination Act* Ensure case files are up-to-date based on the requirements of the FBOP-Statement
  • Key Accountability - Provision of Education and Counseling Services* Perform bi-weekly review of progress on goal attainment to transition to Home Confinement.* Review and document Resident's monthly progress towards goal attainment.* Participates in in-service and competency training. * Prepares special reports as needed on activities, and work group activities.* Meets compliance standards for all documentation and clinical charts.* Provide individualized reintegration guidance.
