How to Sign Up/Activate My Account

To sign up or activate a MyCarle account, all you need is an access code. Get an access code during a doctor’s visit, or by asking at the front desk at any Carle Physician Group location. Your access code will expire in 30 days if you do not activate your account.
If you will need proxy access for a family member or child, you may request a MyCarle proxy consent form when you request an access code. You will be asked to show a photo ID for the consent form.
Set up your account using the following instructions:
- Go to www.mycarle.com and click on “Sign Up Now” on the right of the page
- Enter information for verification
- Access code (from printout provided at physician’s office)
- Zip code
- Date of birth using required format
- Click on the “Next” button
Discard the printout that contains the access code. Once logged in, you won’t need the code again.
- Create a MyCarle ID and password
- Create your password (must be typed twice)
- Create security question and answer
For your security and ease of use, select an ID and a password that is easy for you to remember and impossible for anyone to guess. You will need to enter your ID and password every time you access your MyCarle account.
- Enter your email address (must be typed twice) and click on the “Sign In” button.
- Read the Terms and Conditions
- You must agree to the Terms and Conditions to use MyCarle by clicking the “Accept” button
Terms and Conditions will display each time you log on to MyCarle unless you check the box marked “Please do not show this page next time.”
