Notice of Change of Ownership – Welcome Greater Peoria locations

On April 1, 2023, a number of entities which were formerly part of the UnityPoint Health system became part of the Carle Health system.  During the course of your medical care and treatment you may be given forms or documents which still reflect the UnityPoint logo or contain information referring to UnityPoint.  All documents are in the process of being updated to reflect the new ownership.

The specific entities that are now part of Carle include all former UnityPoint Health entities within the Greater Peoria Area including, but not limited to:

Pekin Memorial Hospital
Proctor Hospital
Proctor Health Systems
UnityPoint Clinic
UnityPoint Health Junction Medical, LLC
Unity Place
Human Service Center
