What We Ask of You:

• Treat all team members, patients and their families with kindness and respect. 
• Questions and discussions are welcome and encouraged - abusive or threatening actions or language are never acceptable.
• Be involved in your care.
• Please follow any specific instructions provided by your doctor when your appointment was scheduled.
• We ask that you at the arrival time listed on your appointment  reminders and in My Carle to allow ample time to check-in for your visit. If you are 10 minutes late to your appointment, it may have to be rescheduled.
• Noncompliance with these guidelines may result in dismissal from the practice. Please speak to your care team for more details
