Family Foundations is a postpartum nurse home-visiting program for women in Champaign County who deliver at Carle Foundation Hospital. We work to ensure both the child and their family’s well-being are taken care of, while connecting them to the necessary community resources.
Through the program, families will receive up to three home visits from a nurse experienced in maternal and child health, breastfeeding and parent-infant bonding.
Community Health Initiatives was developed to work closely with many community cross-sector partners and community members to build an infrastructure and culture of community health, ensuring children and their families, regardless of life circumstances, have an opportunity to be prosperous and live the healthiest life they can.

To join the program or refer someone, call (217) 902-3140. Discuss with your prenatal provider or sign up for visits when you’re admitted to the hospital.
Who Can Participate
Champaign County families with a new baby can participate in the program after they leave Carle Foundation Hospital.
