What is Home Care?
Since 1983, Carle Home Care has provided care for patients in their homes. With close monitoring and patient and family education, many treatments that once kept patients in the hospital now happen safely at home. In addition to the comfort of familiar surroundings, our services are a cost-effective option and are often covered by Medicare, Medicaid and most health plans.

Patients must be homebound, have a doctor's order and insurance approval to be eligible for home health services. Examples of conditions that may require home healthcare are cancer, AIDS, diabetes, heart and lung diseases, fractures, strokes, chronic illness and disabilities.

We are fully accredited by the Accreditation Commission for Health Care (ACHC). This is your assurance that we adhere to the highest standards of quality in the industry.

The Carle Home Care Team
We offer a wide range of services that include registered nurses, certified home health aides, medical social workers and physical, occupational and speech therapists, medical supplies, durable medical equipment and injectable therapies.

We work with you, your family and your doctor to develop a plan of healthcare or treatment to meet your needs. Our Home Care team will:

  • Evaluate and assess your needs and coordinate patient care and services on an intermittent basis with your doctor and other healthcare team members
  • Provide you with continuity from the clinic, hospital or extended care facility into your home
  • Promote a safe home environment for your activities
  • Train you for self-care whenever possible and/or offer your caregivers education so they can manage treatments at home
  • Case manager to use community resources to meet your changing healthcare needs
  • Work toward reaching your highest level of wellness and independence

Pediatric Home Care
You can expect compassionate, patient-centered care for you and your child in the comfort of your own home. Children needing home care and their families are served by specially-trained nurses and physical, occupational and speech therapists. Parents participate in the care and education provided at each home visit.

Home Medical Equipment
You may need medical equipment as part of your home care plan. We'll help you decide what you need and help connect you with a home medical equipment company who will deliver supplies, equipment and oxygen to your home. This can include hospital beds, wheelchairs, respiratory equipment, monitors, walkers and dressing supplies. We have supplies for wound care, catheter care and ostomy care. If you're insured by Medicare-covered home healthcare, all supplies for these treatments and other procedures must be coordinated with your Home Care nurse. Medicare won't reimburse the home health agency for these supplies and won't reimburse you or your supplier. Your Home Care nurse will discuss this with you.
