2024 Community Health Needs Assessment Survey

Carle Health works with community partners to conduct community health needs assessments every three years. This year we will assess Champaign, McLean, Peoria, Tazewell, Vermilion and Woodford counties. 

We use these assessments to put together plans to meet the community health needs. As part of the assessment, a survey is now available for residents of the Illinois counties listed above. The survey takes less than 10 minutes to complete, and all responses are confidential. The surveys will close August 31, 2024. Select the appropriate link below to begin the survey.
