Community Health Needs Assessments

To provide meaningful community benefits, Carle solicits information, ideas and perspectives from the residents of the areas that we serve, including community leaders. Through various forms of research, we develop a community benefit plan. Annually, we report back to the community, relating how we addressed the community health needs identified, having prioritized them for action.

With healthcare reform, this activity by hospitals and healthcare systems is now called the Community Health Needs Assessment (CHNA). Below is the research currently driving our community benefit activities and what will guide our planning and implementation of community benefits.  
 

Carle Foundation Hospital (Champaign County), 2025 Community Health Needs Assessment, 2026-2028 Implementation Plan

The identified community health priorities in Champaign County are:

  • Behavioral Health – Carle addresses behavioral health needs by recruiting additional behavioral health providers, promoting education and training on mental and behavioral health to help reduce stigma, and working with community partners on youth and adult prevention programs.
  • Healthy Behaviors and Wellness – Carle supports activities aimed at promoting health lifestyles by offering prescriptions and programs that promote healthy food options and physical activity, distributing secure and healthy food boxes to families in need, and educating the community on wellness practices through a variety of written distributed materials.
  • Violence – Carle addresses violence by ongoing work with abuse victims through our SANE and CAST programs, promoting police-community relations and increasing community engagement through partnerships with local initiatives and work groups.

Carle Hoopeston Regional Health Center (Vermilion County) 2022 Community Health Needs Assessment, 2023 - 2025 Implementation Plan

The identified community health priorities in Vermilion County are:

  • Behavioral Health - Carle addresses behavioral health needs by recruiting additional behavioral health providers, promoting education and training on mental and behavioral health to help reduce stigma, and working with community partners on youth and adult prevention program.
  • Violence – Carle addresses violence by screening and tracking violent encounters in the hospital, and sharing that data with the community, partnering with the Carle Health system to provide access to care and intervention in sexual assault cases, an providing leadership time to community coalitions that address violence in Vermilion County.
  • Income/Poverty – Carle addresses income and poverty by offering healthcare focused career fairs to students in the community, and healthcare focused job shadowing opportunities at Carle facilities. Carle’s significant commitment to the community, Carle at the Riverfront, will be a natural economic development driver that will drive job opportunities for many.
  • Healthy Behaviors – Carle supports activities aimed at promoting health lifestyles by offering prescriptions and programs that promote healthy food options and physical activity, distributing secure and healthy food boxes to families in need, and educating the community on wellness practices through a variety of written distributed materials.

Previous Needs Assessments
The following are previous Community Health Needs Assessments and Implementation Plans for both Carle Foundation Hospital and Carle Hoopeston Regional Health Center.

 

Carle Richland Memorial Hospital (Richland County), 2021-2023

The identified health needs in Richland and surrounding counties include:

  • Mental Health – Carle Richland plans to address mental health needs for children and adults by continuing the recruitment of behavioral health providers, exploring increasing relationships with inpatient behavioral health and substance abuse providers, increasing access to MAT services and exploring the development of tele-counseling services.
  • Transportation – Carle Richland plans to continue to partner with the local mass transit
    company to utilize their services for patients, in addition to considering the expansion of care
    coordination services to include transportation.
  • Healthy food/low cost recreation – Carle Richland has a demonstration garden in place to assist
    the dietician with healthy food education and plans to expand the project into a community
    garden in the near future. Carle Richland has recently dedicated funds to a local community
    walking path and continues to support local sports recreation options for youth.
  • Dental Care – Carle Richland recognizes dental care as a need, especially for those with no
    insurance or Medicaid, however, CRMH is not equipped to address this need at this time. Carle
    Richland would be interested in partnering with the local FQHC as is reasonably possible to bring
    these services to the community.

Carle BroMenn Medical Center (McLean County), 2023-2025

The identified community health priorities in McLean County are:

  • Behavioral Health – Carle BroMenn addresses behavioral health needs by recruiting additional behavioral health providers, promoting education and training on mental and behavioral health to reduce stigma and by working with community partners on youth and adult prevention programs.
  • Healthy Eating/Active Living – Carle BroMenn addresses healthy eating/active living by collaborating with community partners in offering programs and opportunities supporting active living, increasing access to healthy food and decreasing food insecurity.
  • Access to Care – Carle BroMenn addresses access to care by supporting the local Community Health Care Clinic to ensure added local capacity for health and dental care for low-income residents, by offering mobile health services at various sites within the county and by recruiting more providers into the Carle Health system, thereby expanding access/capacity.

Previous Needs Assessments
The following are Community Health Needs Assessments and Implementation plans for Carle BroMenn Medical Center. Of note, the facility was operated by Advocate Health Care prior to July 1, 2020.
 

Carle Eureka Hospital (Woodford County), 2023-2025

The identified community health priorities in Woodford County are:

  • Mental Health – Carle Eureka addresses mental health by offering adult and youth prevention programs aimed at reducing behavioral health stigma, increasing mental health awareness and/or increasing awareness of adverse childhood experiences (ACEs) and trauma and by working with community partners to increase access to behavioral health services within the community.
  • Access to Care – As the only hospital in Woodford County, Carle Eureka plays a vital role in serving the health needs of a primarily rural area. Community residents benefit from having access to care close to home as provided by a dedicated group of primary care and specialty physicians.
