Champaign County


COVID-19 Vaccine Scheduling Information:
Carle Health, in partnership with Champaign Urbana Public Health District, is vaccinating anyone six months and older who lives or works in Champaign County. We have many available vaccine doses available at this time and encourage those who have yet to receive their vaccine to schedule an appointment today. Appointments for the first or second dose of the COVID-19 vaccine are encouraged.  Appointments are available through MyCarle and the Carle COVD-19 hotline (217) 902-6100.  Those who are 6 months-17 years old are currently only eligible for the Pfizer vaccine.

Coles County

COVID-19 Vaccine Scheduling Information:
Carle Health in partnership with the Coles County Health Department (CCHD) is currently vaccinating those individuals who live or work in Coles County.

Appointments available at Carle Mattoon on Hurst, 2512 Hurst Dr., Mattoon.

  1. For fastest service, anyone 18 and older can schedule through MyCarle. For safety, patients aged 6 months-17 years old must call to schedule to ensure the available vaccine matches the manufacturer’s guidance for administration.

  3. For minors, homebound patients and those needing interpreter services, sign up here for notification of vaccine appointments.

Note – those who are 5-17 years old are currently only eligible for the Pfizer vaccine.

The Coles County Health Department is also offering  vaccines for ages six months old and older.  You can make a vaccine appointment by calling (217) 348-0530. They are open 8:30 a.m. – 4:30 p.m. Monday- Friday. They are closed Saturday and Sunday.

 

DeWitt County Health Department

COVID-19 Vaccine Scheduling Information:
DeWitt-Piatt County Health Department is now offering COVID-19 vaccinations. Please call to schedule an appointment:   

  • DeWitt County residents call (217) 935-3427 ext. 2141 for an appointment at 5924 Revere Road in Clinton.
  • Piatt County residents call (217) 762-7911 ext. 2211 for an appointment at 1020 S. Market St. in Monticello.

Douglas County

COVID-19 Vaccine Scheduling Information:
Douglas County (DCHD) is offering COVID-19 vaccination clinics for the following vaccines.

  • 1st & Booster Dose Johnson & Johnson - ages 18+
  • 1st, 2nd, 3rd/1st Booster, and 4th/2nd Booster Dose Pfizer - ages 12+
  • 1st, 2nd, 3rd/1st Booster, and 4th/2nd Booster Dose Moderna - ages 18+


These appointment-based clinics are scheduled every second and fourth Thursday of the month at the Tuscola Outlet Mall:

Ford County

COVID-19 Vaccine Scheduling Information:
Please visit the Ford County Public Health Department COVID-19 Vaccine Clinic Scheduling page to schedule your COVID vaccine appointments. 

FCPHD will be offering the Pfizer, Moderna, and Johnson & Johnson vaccines along with third doses (if applicable) at the Ford County Public Health Department every Tuesday from 8:30 – 10 a.m. For your second and/or third dose, please bring your vaccination card with you. COVID-19 Vaccine Scheduling Information:
Carle Primary Care locations in Iroquois County are now offering vaccine appointments and encourage those who have yet to receive their vaccine to schedule an appointment today. Call your local Carle clinic to schedule these. Vaccine is open to anyone age 6 months and older.

Jasper County

COVID-19 Vaccine Scheduling Information:
Jasper County is providing vaccine to all persons aged five years and above. You do not have to live or work in Jasper County to receive the vaccine. 

Please call the health department at (618) 783-4436 to schedule an appointment for a COVID-19 Vaccination at the Jasper County Health Department office at 106 East Edwards street in Newton, IL. A parent or legal guardian will need to sign the Vaccine Consent Form for anyone five thru 17 years age.

Lawrence County

COVID-19 Vaccine Scheduling Information:
Lawrence County Health Department (LCHD) is currently vaccinating anyone aged six months old or older. Vaccinations drive-thru clinics are offered at LCHD every Friday (except holidays) from 9 a.m. – 1 p.m.; they offer Johnson & Johnson, Moderna, and Pfizer. Please call the health department at (618) 943-3302 if you need to schedule an appointment for a different time.

Livingston County

COVID-19 Vaccine Scheduling Information:
The Livingston County Health Department (LCHD) is offering Pfizer and Moderna bivalent boosters currently for anyone regardless of whether Moderna, Pfizer, or J&J was received for initial series/boosters. Vaccinations for ages six months old or older are available at the LCHD on Mondays from 8:30 a.m. to 3:45 p.m. (except on government holidays). Appointments are strongly encouraged.

Directions for appointment scheduling and links for registration are available on the LCHD website. LCHD vaccination clinics are open to anyone eligible for the vaccine. A legal guardian must accompany minors to their appointment. You will need to show an ID at the time of your appointment.

For those with limited mobility, a vaccine may be brought to your vehicle during your appointment. Please call LCHD upon your arrival to request vaccination in your vehicle. Livingston County is also working to reach homebound individuals unable to visit COVID-19 vaccination sites. See their website to see if you qualify for the program and a form to schedule an in-home vaccination visit.

Carle Health has many vaccine doses available at this time and encourages those who have yet to receive their vaccine to schedule an appointment today. The COVID-19 vaccine is available to people ages 6 months and above.Patients interested in receiving their first dose of vaccine should contact their primary care provider.

Piatt County Health Department

COVID-19 Vaccine Scheduling Information:
Carle Health is vaccinating anyone 5 and older who lives or works in Piatt County. We have many available vaccine doses available at this time and encourage those who have yet to receive their vaccine to schedule an appointment today. Appointments for the COVID-19 vaccine are by appointment only. Please call (217) 902-6100 or (217) 762-6000 to schedule an appointment. Appointments are available at Carle Monticello, Monday through Wednesday from 8 a.m. – 5:30 p.m. and on Thursday and Friday from 8 a.m. – 4:30 p.m.

COVID-19 Vaccine Scheduling Information:

Richland County TB and Health Office is offering vaccination clinics for individuals age 18 and over each Thursday at their office at 501 S. Whittle Ave. in Olney. You do not need to schedule an appointment; check the public health website for more information.
