The right connections: Client discovers strength, resources through a community health worker

Carle Health Community Health Worker Services create bridges to health and community resources for clients in Vermilion County. Part of the Vermilion County Collaborative, the initiative supports the workers with grant funding from the Illinois Department of Health and Family Services.
