To request your medical records, please print and complete a release form and return to Carle Foundation Hospital.

The Carle Health Information Management Department is located at Carle at the Fields, near the Stephens Family YMCA and Interstate 57. Use Google Maps for driving directions from your location.

How to complete the Authorization to Release Protected Health Information form


Patient Identifiers - this section helps us select the correct patient.

  • Patient Name: Please enter the "patient" s current legal name.
  • Date of Birth: Please print the patient birthdate. Example: 03/04/1956
  • Other Names: Please list any previous names we may have records under. Examples are additional married names, a maiden name, an adoptive name, a name change.
  • MRN: This is the patient medical record number which is also known at Carle as a clinic number.

Where do the records need to go?
This section tells Carle where to send the records.

  • I authorize: Select the location(s) where the visit(s) took place.
  • To Release to: Check this box if Carle should send a copy of the records to another party. Tell us exactly who should receive a copy of the records. Be specific and include a complete address. Please write "Myself" to indicate you, the patient, will be receiving the records.
  • To Request from: Check this box if the requested records are to be sent to Carle from another organization. Be specific and include a complete address.

Method of Release: How should the copies to be sent?

  • Mail - The records will be sent by U.S. Mail to the party listed.
  • Pick up - The records will be held for pick up at the Farber Drive location for 60 days.
  • MyCarle Account - for patient requests only. We can release the requested records to the patient's MyCarle Account. They can be saved or printed using a home computer. The records will be available in MyCarle for 30 days.

Signature and address: Please read the ATTENTION: section. You'll need to sign and date the form. Enter your address at the bottom.

Mental health records may require special authorizations, signatures or releases. Please call our Health Information Management department at (217) 383-3381 for more information.

The authorization will expire one year from the date signed unless otherwise specified. If you have any questions about completing forms or obtaining copies of medical records, call Health Information Management at

 When you express your gratitude through a donation, you help fund many of the services Carle offers today – from state-of-the-art medical equipment to patient-centered facilities to continuing education for staff. Donations can be made to the program of your choice. If the donation honors a member of your care team, we’ll let them know of your generosity, but the amount of your gift will remain confidential. Make a donation now. When you complete a Gold Star, we’ll pass along your gratitude and a special star pin. We’ll also share with their manager and peers recognizing their special effort in caring for you. If you would like to share your experience even further, be sure to mark ‘I give Carle permission to contact me to discuss the possibility of sharing my story with the public’ at the end of the below form and a member of the Carle team will contact you. To give a Gold Star to Carle employee, simply fill out the form.

Recognize a nurse who provided exceptional care to you or a family member.

Do you know a nurse who deserves to receive the Daisy Award? This nurse's clinical skills and especially their compassionate care exemplifies the kind of nurse that out patients, their families and our staff recognize as an outstanding role model. The nurse should consistently meet all of the following criteria:

  • Customer service excellence
  • Community involvement
  • Patient family advocate
  • Educator/mentor
  • Exceptional nursing care

Thank an Employee
Simple gestures of kindness often mean the most to us.

Do you have a compliment or positive story about a doctor, nurse, staff member or volunteer at Carle that you would like to share?

Perhaps you worked with a care team member whose skill and compassion made an impression on you. Maybe it was a kind word or smile that brought you or your family member comfort. Perhaps someone walked you to your destination or spent extra time answering a difficult question.

To thank a doctor, nurse, staff member, volunteer or anyone who made your experience at Carle special simply fill out the form.

When you fill out the form to thank them, we'll pass on your compliment to the employee and give them a special Star pin - and we'll share your story with his or her manager and peers.

You may also make a donation now to honor a health care provider, staff member or volunteer who provided outstanding care. Gifts from grateful patients have funded many of the outstanding services Carle offers today. If a donation is made, we'll inform the team member of your generosity, but the amount of your gift will remain confidential.

Email a Patient

Share your well wishes with a friend or family member in the hospital by sending an email. Simply complete the form below and a Carle Hospital Auxiliary member will hand deliver the email. Messages are delivered once in the morning, Monday through Friday (except for some holidays).

Online Gift Shop

From balloons and flowers, to candy and plush toys, find the perfect gift at the Carle Online Gift Shop. Proceeds are used to offset the operational expenses of the Carle Auxiliary Guest House.

Carle Auxiliary Guest House

The Carle Auxiliary Guest House provides a place of respite for the loved ones of Carle Foundation Hospital critical care patients. The Guest House offers double occupancy rooms and family suites, each with private bathrooms. Inside the home, you will find tranquil spaces to visit and gather as well as a fully equipped kitchen and laundry room. Operated exclusively on charitable donations, the Guest House is offered at no charge to families who need to remain near the hospital while their loved one receives critical care. A referral for an overnight stay is required from hospital personnel, and room availability is not guaranteed. Please call (217) 326-1620 for more information.
