Prepare for Your In-person Appointment

Not sure where to start? Two days before your appointment you will have the opportunity to log into MyCarle   and electronically check-in for your appointment. We will collect information about your medications, medical history, and allergies. If you would prefer, you can use the document below to prepare for your appointment.

Medication List

  • Names and doses of both prescription and over the counter medications

Recent Medical History

  • Chronic illnesses
  • Changes in how you feel or changes in your life

List of complete allergies

  • Food
  • Medications
  • Environment
