Cardiology with Transitional Care
Join us Tuesday, June 18, from 3:30 – 5 p.m. for a discussion about Cardiology with Transitional Care with Sarah Hong-Conder, APRN.
Location: Carle Tower/Heart and Vascular Institute, Conference Room A
Free parking is available in the Heart and Vascular Institute parking lot, just west of the building.
The Cardiac Support Group meets to discuss important heart-health topics. There is no cost to attend and registration is not necessary. Attendees do not need to be Carle patients. Refreshments will be served.
