Short-Term Rehabilitation

Includes skilled nursing and/or rehabilitative care provided by licensed health professionals. These stays are aimed at helping the patient get stronger, usually after a hospital stay, to be successful at home.

Some services provided in Short-Term Rehabilitation include:

  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy
  • Intravenous Therapy
  • Urinary Catheter Care
  • Tube Feeding
  • Complex Wound Treatments

Patients must meet Medicare’s skilled care criteria to be eligible, which includes that the patient requires skilled nursing care on a daily basis and/or skilled rehabilitative services at least 5 days per week. A patient’s length of stay varies based on their progress in therapy and their insurance coverage.

Short-Term Respite Care

Some SNFs offer short-term custodial care to support caregivers. This services allows a caregiver to receive a break from caregiving responsibilities by providing room and board to a patient so that the caregiver may attend to personal matters or go on vacation. During, short term respite care, patients may have limited options to receive physical therapy, occupational therapy and speech therapy under Medicare Part B services.

Long Term Care

Patients needing support 24 hours a day may consider moving into a facility for long term care in order to live safely and securely. These facilities are able to support the patient with activities like bathing, dressing and feeding, cognitive impairment, and certain medical needs.


How to choose a Skilled Nursing Facility

Consider Carle’s Post-Acute Care Collaborative (PACC)

Carle Health partners with local and regional skilled nursing facilities to ensure quality care throughout your rehabilitation. Participation in the collaborative is based on items such as facility’s location and quality of care (which includes evaluating facility’s CMS Nursing Home Compare Star Rating, hospital readmission rate, and skilled nursing facility length of stay). In some facilities, Carle has a healthcare team to follow your care. Please ask one of our social workers or case managers for more information about these facilities.
