What is a Medical Home?
As a member of a medical home, you can expect to be an active participant in your healthcare. In this model of care, you will have access to a full team of providers dedicated to ensuring you get the right care, at the right time. In a Patient Centered Medical Home, care is well coordinated and patient focused.
Your team consists of your primary care physician (PCP), nurse practitioners, physician assistants, registered nurses, medical assistants and other support team members. Your physician is your team leader and ensures all team members are consistently communicating about your medical needs and progress. Your care team communicates continuously to stay informed of yours and other patients' needs and progress.
However, the most important part of Patient Centered Medical Home is you. Your care team is there to know you and your health, and to work with you to make sure that we are meeting your healthcare needs.
