Community Health Needs Assessments

To provide meaningful community benefits, Carle solicits information, ideas and perspectives from the residents of the areas that we serve, including community leaders. Through various forms of research, we develop a community benefit plan. Annually, we report back to the community, relating how we addressed the community health needs identified, having prioritized them for action.

With healthcare reform, this activity by hospitals and healthcare systems is now called the Community Health Needs Assessment (CHNA). Below is the research currently driving our community benefit activities and what will guide our planning and implementation of community benefits through 2019.  

 

Carle Foundation Hospital (Champaign County), 2017-2019

The identified community health priorities in Champaign County are:

  • Behavioral Health – Carle addresses behavioral health needs by recruiting additional behavioral health providers, promoting education and training on mental and behavioral health to help reduce stigma, and working with community partners on youth and adult prevention programs.
  • Obesity – Carle supports activities aimed at reducing obesity by funding programs that promote healthy food options and physical activity, expanding the physical activity prescription program, and providing BMI data to community partners to best measure outcomes.
  • Violence – Carle addresses violence by ongoing work with abuse victims through our SANE and CAST programs, promoting police-community relations and increasing community engagement through partnerships with local initiatives and work groups.

Carle Hoopeston Regional Health Center (Vermilion County), 2019-2021

The identified community health priorities in Vermilion County are:

  • Teen Pregnancy – Carle Hoopeston addresses teen pregnancy through increasing parent involvement and extracurricular activities, promoting sexual education and community programs, and targeting male teens / male involvement to reduce the teen pregnancy rate.
  • Alcohol & Substance Abuse – Carle Hoopeston addresses alcohol and substance abuse by planning and implementing strategies that prevent and reduce substance use and its associated consequences among youth and adults through community and countywide collaborative efforts.
  • Obesity – Carle supports activities aimed at reducing obesity by funding programs that promote healthy food options and physical activity, expanding the physical activity prescription program, and providing BMI data to community partners to best measure outcomes.
  • Mental Health - Carle addresses behavioral health needs by recruiting additional behavioral health providers, promoting education and training on mental and behavioral health to help reduce stigma, and working with community partners on youth and adult prevention programs.

Carle Richland Memorial Hospital (Richland County)

The identified health priorities in Richland and its surrounding 7 county area are:

  • Mental Health – Carle Richland addresses mental health needs by recruiting additional behavioral health providers, promoting education and training on mental health behaviors to reduce the stigma and raise awareness, and working with area partners on youth and adult programs.
  • Dental Care – Carle Richland recognizes dental care is a need, especially for those with no insurance or on Medicaid, however, CRMH is not equipped to provide this service at this time. The hospital would be interested in partnering with the local FQHC if they decide to address this concern.
  • Substance Abuse Resources – Carle Richland continues to support local substance abuse coalitions and other community based programs to reduce substance abuse and its associated consequences among youth and adults.
  • Access to Case Managers – Carle Richland will address the need for case managers by exploring options for access to case managers in both the emergency and clinic settings.
  • Transportation – Carle Richland addresses the need for transportation for patients by continuing to utilize the local mass transit options available, in addition to exploring the option of providing schedules to patients for planning purposes.
  • After Hours/Weekend Care – Carle Richland has addressed this need with the opening of Convenient Care which offers walk-in services to the community and surrounding areas into the evening hours and on weekends.

Carle BroMenn Medical Center (McLean County), 2017-2019

The identified community health priorities in McLean County are:

  • Behavioral Health – Carle BroMenn addresses behavioral health by working with community partners to support adult and youth prevention programs and media campaigns aimed at reducing behavioral health stigma and increasing mental health and substance use awareness, and by working with community partners to increase access to behavioral health services at various sites within the community.
  • Healthy Eating/Active Living – Carle BroMenn addresses healthy eating/active living by collaborating with community partners in offering programs and opportunities supporting active living, increasing access to healthy food and decreasing food insecurity.
  • Access to Care – Carle BroMenn addresses access to care by connecting appropriate emergency department patients with a medical home, by supporting a free clinic which provides dental and medical services to low-income residents and by increasing the use of telemedicine/virtual services.
This community health needs assessment was conducted by AdvocateHealth, when it operated the facility, prior to 2020.

  • Mental Health – Carle Eureka addresses mental health by offering adult and youth prevention programs aimed at reducing behavioral health stigma, increasing mental health awareness and/or increasing awareness of adverse childhood experiences (ACEs) and trauma and by working with community partners to increase access to behavioral health services within the community.
  • Healthy Eating/Active Living – Carle Eureka addresses healthy eating/active living by collaborating with the Tri-County Partnership for a Healthy Community in offering programs and opportunities supporting active living, increasing access to healthy food and decreasing food insecurity.
This community health needs assessment was conducted by AdvocateHealth, when it operated the facility, prior to 2020.
